Master Level Social Worker, School-based

Description:
Social Worker will partner with the Division of Health and Wellness and assigned school to support students receiving educational services outside their assigned school (i.
e.
alternative placement due to code of conduct violation, intensive mental health support in the community, and extended homebound services due to mental health concerns).
The position is intended to provide support to target groups of students who are at an elevated risk due to factors, including history of mental illness, criminal and legal problems, substance use, high conflict and violent relationships, etc.
.
The goals of the program are to reduce the time a student is away from their home school, increase the number of students who are connected to mental health supports, and reduce the likelihood that a student will be placed outside their home school again for the same reason.
Collaborate with the student, parents/guardians, and other relevant parties (i.
e.
, community partners, physician, etc.
).
In-person check-ins may involve home visits or visits to other community sites Assist student and family with making connections to mental health providers, if necessary.
Complete an exit meeting/interview with the student/family to assess Assist in the collection of data for reporting results.
May assist in the provision of professional development for school staff at the school.
May assist in facilitating informational sessions for families and/or community partners.
May facilitate small groups for students (with common needs) to support students with the skills needed to return to school.

License eligibility is required.
Current license is desired.
Experience counseling children and/or adolescents, required.
Must have strong written and oral communication skills, and a strong working knowledge of computers and applications.

Must have a valid driver's license, which meets the requirements of the agency's insurance carrier and reliable transportation.
