The Caregiver Support Program (CSP) is one of the Veterans Health Administration's (VHA) most rapidly growing service areas, as a result of infrastructure modernization, programming innovation, and expansion of population-served by the Program of Comprehensive Assistance for Family Caregivers (PCAFC) under MISSION Act. CSP expansion puts the VHA out front of the healthcare industry as a healthcare system pioneer-caring for the caregiver. The program goal is to not only support caregivers in their role but to also promote their health and well-being through education, therapeutic engagement, and resource and service linkage. The CSP is situated at VA Central Office in 10P4C, Care Management, and Social Work Services (CMSW); operationalized within 18 Veteran Integrated Service System Networks (VISNs); and administered by more than 450 CSP Caregiver Support Coordinators (CSCs), who provide the direct interventions to the program eligible caregivers. Determination of which Veterans and their family caregivers are eligible for the PCAFC, however, is the responsibility of specialized interprofessional clinical review teams. The position serves as a core member of a VISN-level interprofessional group of clinical reviewers-the Centralized Eligibility and Appeals Team (CEAT). The CEAT is responsible for reviewing complex Program of Comprehensive Assistance for Family Caregivers (PCAFC) eligibility cases for admission, ongoing level of care adjustments, and discharge appropriateness. The CEAT is also responsible for adjudicating PCAFC applicant and participant first-level appeals sent from other CEATs within the same region and second-level appeals sent by CEATs based in other regions of the country. The CEAT position requires advanced knowledge and understanding of not only the services and benefits available for caregivers from both federal, state and private sector resources but the statute, regulations, and policy that govern the program. The incumbent must be sensitive to the issues and needs of Veterans and their families, when rendering clinical eligibility determinations, while also ensuring PCAFC applicants and ongoing participants meet the clinical eligibility criteria outlined in CSP PCAFC law and relevant CSO Directives. The position demonstrates expertise in leadership at the systems level that is sustainable. The incumbent uses their experience to demonstrate creative approaches to management of complex situations. Program or service level outcomes must be broad and complex and can be demonstrated at any organizational level within a program, service, facility level, VISN, or VACO. The focus must be broad and complex and at the macrosystem level. The Physician Assistant (PA) incorporates professional standards and implements broad-reaching evidence-based change. Leads and organizes delivery of services to assure continuity and peer accountability for practice. MAJOR DUTIES AND
Responsibilities:
The position is assigned to the VISN within the Caregiver Support Program (CSP), which provides a state-of-the-art continuum of services, education, and resources to caregivers of Veterans with complex needs. As a member of the VISN CEAT, the incumbent has the responsibility for expert knowledge of eligibility criteria for the Program of Comprehensive Assistance for Family Caregivers (PCAFC), day-to-day operation of the program, and all reporting requirements within the assigned VISN. The incumbent performs a wide range of services, collaborating with interprofessional Centralized Eligibility and Appeals Team (CEAT) members to make eligibility determinations, tier level determinations, discharges, and appeals for the PCAFC. Uses clinical judgment in the interpretation of trends and patterns; application of regulation, policy, guidelines; and the recommendation of program enhancements. The role of the PA is significant to the success of a high-performing, high-reliability CEAT, as the PA plays a crucial role in the facilitation of prescribed medication and treatment, care coordination, advocacy, and education. The PA's training in assessment and monitoring, diagnosing, treating, health restoration, wellness promotion, illness prevention, and coping facilitation is crucial to the CEAT's case review. The PA's knowledge of disease management, special diet plans, and medical conditions, medication prescription and administration, self-care completion, and caregiver education helps ensure CEAT clinical determinations are accompanied by comprehensive, whole health care plan recommendations, for the Veteran's facility healthcare team to consider for safe and quality care continuity. Work Schedule:
Tour of duty is varying daylight hours, Monday through Friday; however, weekend and evening hours may occur when Veteran needs arise. This position requires travel to facilities within VISN and may require extended work hours to facilitate travel. Financial Discloser Report:

Graduate of a PA training program, or of a surgical assistant program which is accredited by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) or one of its predecessor organizations; and A Bachelor's degree from an accredited PA training program or in another health sciences related field from an accredited college or university recognized by the U.S. Department of Education. Current and continuous certification by the National Commission on Certification of Physician Assistants (NCCPA) is required for employment in VHA. Physical Standards:

Experience:
An active state license as a Physician Assistant and possession of advanced practice skills and judgment demonstrating a high level of professional competency in managing assignments/projects of a broad scope, magnitude and complexity. Substantial experience in clinical administration is strongly desired. Clinical experience in primary care, urgent care and/or emergency care is highly desirable. Grade Determinations:
This position is boarded by the VISN or Physician Assistant Professional Standards Board. Clinical privileges aligned with the function of a physician assistant integrated with the CSP commensurate with the practitioner's training and experience will be granted by the VISN or VA medical center.
