Registered Nurse - Polytrauma / Spinal Cord Case Manager

The Polytrauma(TBI)/Spinal Cord Case Manager will; Develop an assessment of the veteran in collaboration with other providers, the veteran, family members and significant others. The goal of the assessment is to highlight the veteran's strengths, limitations, and internal/external supports and service needs in order to optimize the veteran's functional status. Contact the Veteran and Servicemember prior to transfer for inpatient admission and the first outpatient appointment to answer any questions about upcoming rehabilitation procedures; Demonstrates an advanced understanding of the range of treatment and skills needed for all adult patients including geriatric patients related to their physical limitations, psychological needs and age associated illnesses and conditions. The incumbent must have an advanced level of clinical knowledge of the complex problems associated with a traumatic brain injury and be able to provide current and advanced techniques to the treatment of this veteran population. Be the subject matter expert on VA and/or community resources with the ability to provide training, orientation, consultation and guidance within clinicalspecialization of practice. The incumbent will handle all Polytrauma consults and place follow up consults when appropriate. Communicate and coordinate the care plan with the IDT providers, other involved case managers, and the Veteran, Servicemember, and their families. This includes regular communication with the TCM (OEF/OIF/OND) Program Manager, and participation in TCM (OEF/OIF/OND)Care Management team meetings and activities within the facility; Provide education related to VA and community resources, entitlements, Advance Directives/Living Will and will refer veterans/families to the appropriate interdisciplinary team member for identified health education needs. Coordinate treatment and communicate with other social workers and community program coordinators to include HHA, CNH, and TCM (OEF/OIF/OND), PTSD, Homeless Program, Suicide Prevention etc, to facilitate continuity of care for veterans. Assist the veteran/family in establishing the appropriate referrals based on the veteran's preference or that of his surrogate decision-maker. Incumbent will develop relationships with community partners who can support and treat Veterans with traumatic brain injuries. Educate and encourage the veteran/family to advocate on own behalf, thus fostering a sense of independence and empowerment. Have experience in making rapid assessments and developing crisis management plans. The incumbent has access to multiple resource directories in addition to previously developed resources to meet the demands of a crisis. Be responsible for staying informed of current theory and treatment modalities by attending conferences, clinical seminars related to the treatment of traumatic brain injury and complex mental health disorders. Review the progress notes from the other providers to accurately determine the strengths and limitations of each veteran being referred for services. Fulfill ethical responsibilities to Veterans, colleagues and VA in accordance with relevant discipline Code of Ethics, VHA Code of Ethics, and local Ethics policies, which includes but is not limited to compliance with all laws and regulations related to confidentiality and release of information, and protecting Veterans' personal identifying information. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Experience:
2 years outpatient nursing experience preferred. Grade Determinations:
The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
